What a human-readable record of an AI decision must contain to hold in front of a regulator, a buyer, or a board. Free to adopt, with or without us. A standard only matters if it is bigger than its author.

Readable in one pass by someone who was not in the room.
- The system that produced it.
Which model, which version, which workflow. Linked, not implied.
- The approval.
Who signed, when, against which written policy.
- Written once, never edited.
Corrections are new entries. History has no delete key.
- Independently verifiable.
The chain must be checkable by the reader's own engineer, with tools they control.
- Kept beyond the tool.
The record outlives the vendor, the platform, and the contract that made it.
- Answerable in minutes.
If the question comes, the answer is retrieved, not reconstructed.
